Exemption - Checklist required for Exemption for Courtesy Credits for Producer-Related functions occupied by Non-Canadians (This does not apply to Treaty Co-Productions)

1. Signed Affidavit: in CAVCO-approved form, by the non-Canadian individual requiring an exemption for courtesy credit.
   
   
2. Agreement for Person to be Exempted: executed copy of the agreement(s) indicating the duties performed by the individual (executive producer, associate producer, supervising producer, creative consultant, etc.).
   
   
3. Preliminary Credits: an example of the format in which the courtesy credit(s) will appear in the final credits.

The Office may ask for any other documents which are deemed necessary to assess the application. Agreements must be signed.
