Declaration re Waiver

1. a Waiver in Respect of the Normal Reassessment Period (CRA form T2029), as described in subparagraph 152(4)(a)(ii) of the Income Tax Act, HAS BEEN FILED with the Canada Revenue Agency within the reassessment period applicable for the above-mentioned corporation for the fiscal year(s) ending _________________________ and __________________________ (if applicable) for the purpose of claiming the Canadian Film or Video Production Tax Credit (CPTC) in respect of the production entitled _________________________________________________________, CAVCO PCH number _______________ (PCH number assigned by CAVCO at the Part A application stage, if applicable).

2. a Waiver in Respect of the Normal Reassessment Period (CRA form T2029), as described in subparagraph 152(4)(a)(ii) of the Income Tax Act, HAS NOT BEEN FILED with the Canada Revenue Agency for the above-mentioned corporation for the fiscal year(s) ending _________________________ and _________________________ (if applicable) for the purposes of claiming the Canadian Film or Video Production Tax Credit (CPTC) in respect of the production entitled ____________________________________________________, CAVCO PCH number _______________, because the Canada Revenue Agency has not yet assessed the income tax return(s) of the above-mentioned corporation for the fiscal year(s) indicated herein.
